How Do I Place Consent Forms at the Bottom of an Intake Package?

By default any consent forms you attach to an intake questionnaire will display at the top (beginning) of your intake package, before any other questions. To move the consent forms to the bottom (end), follow the steps below:

1. Navigate to "My Forms" and open the intake questionnaire you'd like this changed on.
2. Click on "... > Settings".
3. Check the box for "Show consent forms at the bottom, instead of top".
