Duplicate a Form

Follow the steps below to duplicate a questionnaire, consent form, or treatment note in PracticeQ.

Duplicate the Form

1. Click My Forms and open the form you wish to duplicate.
2. For intake questionnaires and treatment notes, click the three dots icon and select Duplicate. 
3. For consent forms, click Duplicate from the menu on the left. 
4. Enter a new name for the copied form and click Create Copy.
5. That duplicated form will now be in your account under the name you gave it.

Change the Form Type

When duplicating a questionnaire or treatment note, you can change its type before saving. This option is not available for consent forms.
