How to Duplicate a Questionnaire or Consent Form

To duplicate any type of questionnaire or consent form, follow the steps below:

1. Locate the form you want to duplicate under the "My Forms" menu.
2. Click on the form to open its editor.
3. For intake questionnaires and treatment notes click on "... > Duplicate", for consent forms click on "Duplicate" in the left menu.
   

   

4. Enter a new name for the copied form and hit "Create Copy".
5. That duplicated form will now be in your account under the name you gave it.

Changing Form Type

When duplicating an intake questionnaire or treatment note, you can choose to change the type it is, as shown in the screenshot below:
