Form Under Review
If one of your forms is showing the message “This form is under review,” see the information below to determine the cause and resolve the issue.

Common reasons forms are placed under review by PracticeQ
- The form requests credit card information
- Forms cannot request credit card information unless it is collected securely through a connected payment provider, such as PracticeQ Payments.
- The form requests Social Security Numbers/Social Insurance Numbers
- PracticeQ does not recommend using your forms to gather this information from your clients.
- The form contains "spammy" language

Steps to Resolve
- Remove the questions requesting credit card information, social security numbers, or social insurance numbers.
- Edit or remove any other parts of the form referring to these topics, including Section Title/Note items.
- Edit questions and sections to remove spammy language.
- Wait for PracticeQ to perform its next system scan, then try using the form URL again. The scans are performed hourly.
