Clients Can't Access My Form Anonymously – What's Wrong?

If you added a form to your website and cannot access it, you may have missed an important step during setup. 

  1. Click My Forms and open the form you wish to add to your website.
  2. Click More → Get URL.
  3. Check the box labeled I want this intake form to be accessed through my website.
  4. We also recommend that you check the box labeled Require email or phone confirmation, especially if the form requires e-signatures.
  5. Use the given URL to create a link on your website. If you do not know how to create an HTML link on your website, provide it to your web designer or contact us at hello@intakeq.com for assistance.
