Prevent Practitioners from Editing Questionnaires and Consent Form Templates

Admin accounts can make form templates read-only to prevent editing by other team members.

Prevent other practitioners from editing a questionnaire

1. Click My Forms and open the questionnaire.
2. Click the three dots icon and select Settings.
3. Check the box labeled Prevent other practitioners from changing this form.

Prevent other practitioners from editing a consent form

1. Click My Forms and open the consent form.
2. Click the Access Control link.
3. Check the box labeled Prevent practitioners from changing this form.
