If you wish to export more account information - such as portal communication, file attachments, and treatment notes - request an account export! 

Price

• The cost of an account export is $75.00. 
• This is a flat fee for all accounts.

Time Frame

• An account export typically takes 1-3 business days to prepare. 

Delivery and Format

• Our team will provide a URL to download the export file when it is complete. 
• The export will be a .zip file containing a folder for each client with their name and client ID.  

Contents

Each client folder in the .zip file will contain the following:

• All questionnaires and consent forms in PDF format
• All treatment notes in PDF format
• All file attachments from the client's timeline, in the same formats as the original files
• All client portal communication (chat history)
• Staff comments

• Contact us at hello@intakeq.com to make your export request.
• The team will ask you to confirm the last 4 digits of the credit card on file for your IntakeQ account. 
• Once the team verifies the card on file, they will begin the export.
