Submit a Form to the Form Conversion Team

Would you like our Form Conversion Team to convert your form for a small fee? You can still submit it—we’re happy to help!

While the Upload Existing Form option is no longer present in the Questionnaires and Consent Forms sections, you can still find it on the My Forms page.

1. Click My Forms.
2. Scroll down to the Note Templates section.
3. Click Upload Existing Form.
4. You will be directed to the Forms Conversion page you are familiar with. 
5. Upload your questionnaire, consent form, or note template here, and the Form Conversion team will begin the conversion process!
