Change the Order of Consent Forms

To change the order in which consent forms are displayed within an intake questionnaire package, follow the steps below.

1. Click My Forms and open the intake questionnaire.
2. Click Consent Forms.
3. Uncheck all the consent forms that have been previously checked.
4. Re-check the consent forms in the order in which you want them to appear.
5. Click OK.
