How to Disable or Edit the Default Note

Each questionnaire starts with the following note at the top of the form, by default:

To Disable the Default Note:

1. Click on "My Forms" and open the form template you wish to edit
2. Click on "... > Settings".
3. Check the box, "Hide Default Instructions".

If you want to add your own note, you can use the "Form Intro" setting.

To Edit the Form Intro:

1. Open the form you want to edit under the "My Forms" menu.
2. Click on "... > Settings".
3. Open the "Advanced" tab.
4. Scroll down to find the "Form Intro" text editor and enter the text you want to show up at the top of your form.
