Add Placeholders to Consent Forms

PracticeQ supports the use of placeholders in consent forms templates. 

Placeholders are like variables used in form templates, such as [PractitionerName]. They are replaced by real data when a client form is created.

The following placeholders are currently supported:

  • [ClientName] - will be replaced by the client's full name.
  • [ClientFirstName] - will be replaced by the client's first name.
  • [PractitionerName] - will be replaced by the full name of the practitioner associated with the client form.

Add a Placeholder to a Consent Form

  1. Click My Forms and scroll down to the Consent Forms section.
  2. Open the consent form that requires a placeholder.
  3. Click the blue Placeholders link on the top right.
  4. The available placeholders will display; copy and paste the ones you wish to use into your Consent Form template.

Example

The following text in your consent form template:

"I understand that [PractitionerName] from Heathy Clinic Ltda. will have access to my health records."

will turn into:

"I understand that Dr. Joseph Smith from Heathy Clinic Ltda. will have access to my health records."
