Add a Form Intro

The Form Intro field allows you to add any formatted text to the top of your forms. Since it supports HTML, you can have richly formatted text, including links, images, and embedded videos. It's ideal for welcome messages and instructions.

Note: Unlike the Section/Note question type, which also allows you to add rich text to your forms, the Form Intro is placed on top of all other content, with the exception of the form header.

Create Your Form Intro

  1. Go to the My Forms menu.
  2. Select the form to which you wish to add a Form Intro.
  3. Click the ... icon and select Settings from the dropdown menu.
  4. Open the Advanced tab.
  5. Scroll down to Web Form Settings.
  6. Locate the Form Intro text editor.
  7. Enter your Form Intro content.
  8. Click the Add Field tool to insert placeholders. Placeholders will populate with the pertinent data once the client logs into the form. Available placeholders are:
    1. [ClientName]
    2. [ClientFirstName]
    3. [PractitionerName]
  9. Click Close.
