Provider Signature in Consent Forms

Some consent forms need to be signed by both the client and the provider. In these cases, it’s possible to indicate that a consent form requires the provider's signature.

Prepare the Consent Form

  1. Click My Forms and scroll down to the Consent Forms section.
  2. Open the Consent Form that requires both the client and provider signatures.
  3. Check the box labeled Enable Provider e-Signature
  4. If you wish to add your own text above the signature, click the Customize link.

After the Client Submits the Form

  1. Click Lists → Intakes.
  2. Locate the consent form; you will see a caution sign next to it stating Your signature is pending.

  3. Click View to open the form.
  4. Click the orange Sign button.
  5. Click the Click to Sign button encased in red arrows.
  6. Your saved signature will populate on the form.

Print the Consent Form

  • When printing the consent form, the provider signature will appear under the client's signature.
