Mapped Demographic & Insurance Questions

When you start creating a form one of the first things you'll typically want to capture is a client's demographics. IntakeQ has a pre-built question for this, which will add all relevant profile fields, and automatically map them so they populate on a client's profile page.

We also have pre-built questions to capture a client's primary and secondary insurance information, which would be used for superbills or claims.

To add these pre-built questions, see the screenshot below:

As you can see above, the three options you can add are available once you click on the three vertical dots to the right of the form's name. Below are examples of what will be populated into your form when you select each question type.
