Deactivate a Form Template

Deactivate a form template; this removes it from your dropdown menu while keeping it in your system. 

Deactivate a Template

1. Click My Forms. 
2. In the Questionnaires section, locate the form you wish to disable.
3. Click the three dots icon in the upper right corner of the form icon and select Deactivate.
4. Click Yes to confirm.
5. The form template will now be disabled. It will appear lighter in the list and will display an inactive tag.

Notes: 

• Any pending forms using this template that were already sent to clients will still function normally.
• After deactivation, clients will not be able to access this form via a public URL.

Reactivate a Template

1. Click My Forms. 
2. In the Questionnaires section, locate the form you wish to reactivate.
3. Click the three dots icon in the upper right corner of the form icon and select Reactivate.
