Legal Names on Insurance Documents

In some instances you may want to capture a clients preferred name as the main name on their profile, but need a different "legal name" on insurance documents. This is supported in IntakeQ by setting a legal name for the client, which will then automatically be used on superbills or claims.

To set a legal name on a client, follow the steps below:

  1. Open the client's profile via "Lists > Clients" or the search bar.
  2. Navigate to the "Claims" tab of the client profile, and click "Claim Defaults".
  3. On this screen you'll see an option labelled "Patient Legal Name", this is where you enter their legal name.

  4. Be sure to click "Save" when you're complete.

That is all that is needed, once that is set the legal name will be used on the insurance documentation going forward as opposed to the main name set on the client's profile.
