Enhanced Questionnaire Customization

PracticeQ offers enhanced ways to customize your intake questionnaires. 

Select unique headers and email templates for each form, decide where your consent forms will appear in your Intake Packages, and hide progress indicators.

Custom Email Template per Questionnaire

  1. Go to My Forms.
  2. Open the form you wish to edit.
  3. Click the three dot icon and select Settings.
  4. Select the Invitation Email or Other Messages tab.
  5. Check the box that says Use a custom email template for this form or Customize Form reminder Email for this form.
  6. Create your template.

Note: Please leave the [FormLink] placeholder in your template. The placeholder will be replaced by the link and password your client will use to access the form.

Custom Header per Form

There may be situations where using a different header for a specific form makes sense. Maybe you offer different services at a different location, or use a different branding for certain services.

Your global form header will be defined in More → Settings, but you can override it on a form-by-form basis.

  1. Click My Forms.
  2. Open the form you wish to edit.
  3. Click the three dots icon and select Settings.
  4. Select the Custom Header tab.
  5. Check the box labeled Use a different custom header for this form.
  6. Create your form header.

Consent Forms Position

Consent forms are located at the top of Intake Packages by default, but if you wish, you can place them at the bottom, after the last question.

  1. Click My Forms.
  2. Open the form you wish to edit.
  3. Click the three dots icon and select Settings.
  4. Check the box labeled Show consent forms at the bottom, instead of top.
  5. Click Close.

Progress Indicator and Default Instructions

PracticeQ allows you to hide the progress indicator (typically located at the bottom of the screen) and the IntakeQ default note located at the top of the form.

  1. Click My Forms.
  2. Open the form you wish to edit.
  3. Click the three dots icon and select Settings.
  4. Check the box labeled Hide progress indicator and/or Hide default instructions.
  5. Click Close.

Notes: 

  • The default note says “Fill out this questionnaire at your own convenience. Your answers are saved as you go, so you don’t need to finish it all in one sitting. Submit the answers only when you have answered all that you can“.
  • If you choose to hide the default note, you can create your own message by using the Section Title question type.
