Create a Custom Header for PDF Forms

With PracticeQ, you can create a custom header to be used only for the printed version of your forms. This is useful when you have colorful or flashy headers in your web forms, but you prefer to display a more formal and discreet header when the document is printed.

Set up your custom header

  1. Navigate to the More menu.
  2. Click Settings.
  3. Select PDF Header.
  4. Choose the Custom radio button.
  5. Enter the custom PDF header in the text editor that displays. You will see a preview of your entry below.
  6. If you would like this header to display in Treatment Notes as well, place a checkmark in the box above the text editor.
