Form Conversions: Requirements and Pricing

The IntakeQ team can convert your paper forms for you. Upload your forms or email them to hello@intakeq.com and the team will respond with a price quote!

Requirements for submitted forms:

• Include only one form per file
• ❗️IMPORTANT: The name of your file will become the title of your Form Template.
• Digital forms are preferred; scanned pages cost more
• Do not include instructions – send exactly what you want to appear in your template
• Only MS Word and Adobe PDF files are accepted
• Form must be in preferred language; translation service is not available
• We strongly advise against making a questionnaire as a consent template, or vice-versa. The resulting template will not function as intended in our system.

Pricing:

Standard files (copy/paste)

• $9 per page for up to 10 pages 
• There will be a $2 charge per page after the minimum is reached 

Scanned/encrypted files (no copy/paste)

• $50 for up to 5 pages
• There will be a $15 charge per page after minimum is reached
