Request Edits to a Converted Form

When you receive a converted form from the IntakeQ Team, you have a 30-day window to request a free edit.

• You will receive one round of revisions in the first 30 days at no extra cost.
• It is your responsibility to review converted form templates within the first 30 days of completion and notify the IntakeQ team of minor errors, such as typographical errors or small omissions. Note: If major modifications are needed, you will be subject to additional fees.
• Any request for an IntakeQ representative to edit a form template after 30 days of creation is subject to additional fees, based on the amount of time required to perform the changes.
• If you would like a Native PDF consent template to be edited, please edit your original form first. Then submit the file to us (via email only: hello@intakeq.com). There is normally no charge for this.
• All form edit requests must be submitted in writing. Please include:
  • The full template title, as seen on the My Forms screen
  • The section of the template where the error is located
  • For questionnaires, please include the question number seen from the editing screen, not from the preview
