PracticeQ Payments: Why is My Application Pended?
After you apply for PracticeQ Payments, you may see that your application is pended. Here is a guide to managing your pended application.
About Pended Applications
- The processing team will pend your application if they need more information from you; you will receive an email advising you that your application is pended from hello@intakeq.com.
- Your pended application will expire in 90 days.
- Upload documents or update your application under Booking Settings → Payments.
- If you need to comment or answer a question about your application, you can do so by uploading your comment in a word/text document in the Payments tab.
- Further updates from the team will come within 24 business hours. Please look at the topmost information for the latest update on your Application Status:

Possible Reasons for your Pended Application
- Improper/incomplete ID upload
- Your application must include an upload of a valid, government-issued ID.
- The upload must be a clear, color photo of the ID - not a scan.
- The address on the ID must match the signor's home address.
- Incomplete/invalid Address verification document
- The address verification document must have the signor's name listed. Acceptable documents include:
- Articles of Incorporation or Organization
- Annual filing from SOS or Certificate of Good Standing
- Partnership Agreement
- Business license/permit
- Tax license
- Most recent business tax return (filed)
- Fictitious name filing
- Schedule K-1
- Assignment of Interest document (when an agency/lawyer filed on behalf of the owner(s) with the SOS
- Medical license not included
- Incomplete Bank verification document
- Your application will typically require either a voided check OR a bank letter:
- If providing a voided check:
- MUST include the DBA name or company name
- CANNOT be a starter cheque, digital cheque, or have penned in company name/address.
- MUST have "VOID" handwritten
- MUST be a clear, color picture of the check - not a scan
- If providing a bank letter:
- MUST be on Official Bank letterhead.
- MUST include the Bank Name and Branch Address.
- MUST have Merchant Legal Name AND DBA Name.
- MUST include the account number.
- MUST include the routing number.
- MUST include a signature and contact telephone number of an employee of the bank.
- MUST include "this account is active and is in good standing with the bank".
- MUST be dated and less than 90 days old.
- Lack of Marketing materials
- Your application must include marketing materials to attest to the fact that you are a valid business
- Experian credit frozen
- The processing team needs to do a soft check of your credit as part of the application process. If your Experian account is frozen, please ask for a temporary unfreeze.
