Use mapped labels in your consent forms to populate known information and to minimize staff and client work.
Add a Mapped Label
Click My Forms and open the consent form you wish to edit.
Place your cursor where you want the mapped label to appear in the body of the consent form.
Click Insert Field → Mapped Label. (For Native PDF consent forms, click and drag the Mapped Label tool into the document.)
Check Is Required if you wish; this is optional.
In the Profile Mapping field, you must set the mapping, or PracticeQ will have no information to pull.
Benefits of Mapped Labels
The data pulls directly from the profile, so all mappings are supported.
Mapped labels show data in ways otherwise not possible, such as gender as text. They allow you to do this without having to create a manual radio button setting.
You may even use mapped labels for dropdown and multi-select dropdowns, making them ideal for custom set ups!
