Use Mapped Labels in Consent Forms
Use mapped labels in your consent forms to populate known information and to minimize staff and client work.
Add a Mapped Label
- Click My Forms and open the consent form you wish to edit.
- Place your cursor where you want the mapped label to appear in the body of the consent form.
- Click Insert Field → Mapped Label. (For Native PDF consent forms, click and drag the Mapped Label tool into the document.)
- Check Is Required if you wish; this is optional.
- In the Profile Mapping field, you must set the mapping, or PracticeQ will have no information to pull.
Benefits of Mapped Labels
- The data pulls directly from the profile, so all mappings are supported.
- Mapped labels show data in ways otherwise not possible, such as gender as text. They allow you to do this without having to create a manual radio button setting.
- You may even use mapped labels for dropdown and multi-select dropdowns, making them ideal for custom set ups!
