Send a Consent Form to a Client

PracticeQ allows you to send only a Consent Form to a client, without needing to create an intake package.

Send a Single Consent Form

  1. Click My Forms and scroll down to the Consent Forms section.
  2. Open the consent form you wish to send.
  3. Click the Send to Client link from the menu on the left. 
  4. Type the client’s name and email.
  5. Click Send Form.

This will automatically create an intake package containing a single consent form.

URL Option

You may wish to place a consent forms public URL on your website:

  1. Click My Forms and scroll down to the Consent Forms section.
  2. Open the consent form.
  3. Click the Get URL link from the menu on the left. 
  4. Check the box labeled I want this form to be accessed through my website.
  5. If desired, click the box labeled Require email or phone confirmation. For more information on this feature, read Require Email/Phone Confirmation for Your Public Forms.
  6. Click copy to copy the form URL.
  7. Copy the HTML link if needed.

Note: If you wish to send multiple consent forms to a client at once, you will need to create an intake package. The article Create an Intake Package will walk you through this.
