Create a Form PDF

Save a form as a PDF for emailing, printing, or faxing.

Questionnaires

1. Click My Forms and open the questionnaire you want to save as a PDF.
2. Click the three dots icon and select Print PDF.
3. Use the File menu in your browser to print or save the PDF file.

Consent Forms

1. Click My Forms and open the consent form you want to save as a PDF.
2. Click the Open PDF button from the menu on the top left.
3. Use the File menu in your browser to print or save the PDF file.

Treatment Notes

1. Click My Forms and open the treatment note you want to save as a PDF.
2. Click the three dots icon and select Print PDF.
3. Use the File menu in your browser to print or save the PDF file.
