How to Save a Questionnaire, Consent Form or Treatment Note as a PDF

If you would like to save any of your forms' PDF to your computer (for emailing, printing, faxing, etc. purposes), follow these steps:

Intake Questionnaires

1. Navigate to "My Forms" and select the intake questionnaire you want to save as a PDF.
2. Click on "... > Print PDF".
3. A new screen with the PDF preview will show; click on the download button on the top right to save the file to your computer.

Consent Forms

1. Navigate to "My Forms" and select the consent form you want to save as a PDF.
2. Click on the "Open PDF" button (top left of content box).
3. A new screen with the PDF preview will show; click on the download button on the top right to save the file to your computer.

Treatment Notes

1. Navigate to "My Forms" and select the treatment note you want to save as a PDF.
2. Click on menu "... > Print PDF".
3. A new screen with the PDF preview will show; click on the download button on the top right to save the file to your computer.
