Create an Intake Package with Consent Forms Only

When clients only need to sign a consent form bundle, create an intake package containing consent forms only.

To create an intake package that contains only consent forms (no questionnaire), follow the steps below.

1. Click My Forms and navigate to the Questionnaires section.
2. Click the green box + Create New.
3. Enter a Form Name.
4. Click Continue.
5. You now have an empty intake package that does not contain a questionnaire. 
6. We recommend adding a field with instructions. Click the green Add New Question button.
7. Select the question format Section Title/Note and in the Section Text (Optional) box, type the instructions you wish to give your clients.
8. Click the Consent Forms button and select the consent forms you want to include in your package. Note: They will appear in the order you select them.
9. Click OK.
10. Click Preview to see what the form will look like to your clients.
11. Click Send to Client, or three dot icon → Get URL if you want to place the link to the forms on your website.
