How Do I Send an Intake Form to a Client?
To send an intake form to a client, follow the steps below:
- Log in to IntakeQ.
- On the dashboard, click on the big orange button named "Send Form to Client".
- Select the appropriate form from the dropdown list.
- Type the client’s name and email.
- Optionally, you can add a note to your client. This note will be displayed above the first question of the intake form.
- Click on "Send Form".
That’s it. You’ll be notified when your client completes the form.
If you need to see the form you just sent, click on "Lists > Intakes" and search for the client name.
You can also send forms automatically via your booking widget, through the Secure Messaging System, through scheduled forms before/after an appointment, by adding forms to your website, or by placing a link to forms on an iPad/tablet device.
