Send an Intake Form

This article explains how to send an Intake Form to a client. Add personalized instructions to your form and check its status after sending.

Send the Intake Form

  1. Navigate to My Dashboard
  2. Click Send Form to Client.
  3. Select the form you wish to send from the Intake Form dropdown menu.
  4. Type the client’s Name and Email. If the client is already in the system, their name and contact information will populate in a dropdown. See related article Add a Client if needed.
  5. In the Send to Client dropdown menu, select Email or SMS to dictate how the form will be sent to the client.
  6. Add Instructions for the client; this is optional. The instructions will display above the first question of the Intake form.
  7. Click Send Form.

Confirm the Status

  1. Return to My Dashboard.
  2. Look in the Pending Forms pane; if the form sent successfully you will see it listed there. Your action will be recorded under Latest Account Events as well.
  3. To view the status of the form, go to Lists.
  4. Select Intakes.
  5. Search by client name or review the list of recent activity.
  6. Note the Status and Completed columns for the form.
