Practitioner's Notes on Intake Forms

IntakeQ allows you to add one note per question on an intake form after your patient has completed it.

To add your own notes, open a completed questionnaire, hover the mouse over a question and click on the Add Note button.

Once you add your note to a question, it will show up in red, like this:
