Customize Your Form Email Template and Header
PracticeQ allows you to create a global form invitation email. However, if you wish to use a different email template for a particular form, you may override the global template in the form settings.
Create a Custom Email Template for a Form
- Click My Forms and select the intake questionnaire you want to edit.
- Click the three dot icon and select Settings.
- Select the Invitation Email tab.
- Check the box labeled Use a custom email template for this form.
- Create your template.
- Click Close.
Note: Do not remove the [FormLink] placeholder in your template. This placeholder will be replaced by the link and password your client will use to access the form.
Create a Custom Header for a Form
There may also be times when using a different header for a specific form makes sense. For example, you may offer different services at a different location, or use different branding for certain services.
- Click My Forms and select the intake questionnaire you want to edit.
- Click the three dot icon and select Settings.
- Select the Custom Header tab.
- Check the box labeled Use a custom header for this form.
- Create your form header.
- Click Close.
