Form Template Ownership
How does PracticeQ determine which practitioner owns a particular questionnaire or consent form?
Click Lists → Intakes and filter the report by Practitioner. Not sure what assigns a form to a practitioner? Read below for more information.
Possible Form Ownerships
- The form template lives in the admin account and is shared with all practitioners by default.
- The form template lives in the admin account and is not shared across practitioners.
- The form template lives in a single practitioner’s account and is not shared with anyone.
Set sharing permissions on a per-template basis; open the template and click the three dots icon → Settings.
Ownership Upon Sending
- If the form template lives in the admin account and it is shared, then it will be owned by the person who sent the form.
- If the form template lives in the admin account but it is not shared, it will be owned by the admin.
- When an assistant sends a form that is not associated with an appointment, the form will be owned by the practitioner the assistant manages.
What Affects Form Ownership?
- Client Timeline: Form ownership is not determined by the owner of the client timeline.
- Sender: The user who sent the form can be a factor; read the section above.
- Appointment:
- The practitioner associated with the appointment will own the form.
- Questionnaires may be assigned to specific appointments; in that case, the practitioner the appointment is scheduled under will own the form.
- Rules: Rules may be applied to the form that change its owner. To see if rules are in place, open the questionnaire, click the three dots icon and select Rules.
- Form URLs: If the sender selects the Get URL option in the form editor, they can select a practitioner to route the form to.
Can Form Ownership be Changed?
- To change the owner of a completed form, open the form and click More → Transfer.
- To change the owner of a form template, follow the directions here: Transfer Form Templates Between Practitioners.
