Get the Public URL of an Intake Form
Obtain the public URL of an intake questionnaire or form package you so can integrate your forms with your website or easily share them with clients.
- Click My Forms and locate the Questionnaires heading.
- Open the intake form you want to add to your website.
- Click the three dots icon → Get URL.
- Check the box labeled I want this intake form to be accessed through my website.
- IMPORTANT: In order for mapped fields to populate forms automatically, you must check Require email or phone verification under Additional Options. However, if you are using devices in your office, you can choose Enable Secure Office Access.
- If your organization has more than one practitioner, choose which practitioner submitted forms should be routed to.
- Use the given URL to create a link on your website or add to an email.
