Scored Assessments as Questionnaires

Providing your patients with comprehensive mental health care requires precise tools and timely insights. PracticeQ can get you started with your care faster by providing you access to scored behavioural health assessments integrated seamlessly into your workflow. The assessments will automatically be scored and you can record them separately to be viewed over time.

Assessments That Need Manual Scoring 

There are assessments in the application that are not able to be scored automatically at this time, and will need to be manually scored.

Importing Assessments from Standard Forms to 'My Forms'

You may already have these in the "My Forms" page. If not, you can download them from the Standard Forms section:

  1. Login to your IntakeQ account and click on "My Forms" in the blue banner at the top.
  2. Click on the "Standard Forms" button in the upper right corner of the page.
  3. Browse the list of specialties, and click on the icon under preview to see how the form will look. Click "Import" on the one you'd like to add to your account.
  4. You will see that this assessment template will now appear in your account under "My Forms

Sending Assessments to Your Clients 

Via the Client Timeline

  1. Go to a Client's timeline and click the "+" icon on the top right.
  2. Select "Send Intake Form" and select the assessment you want to send
  3. Click "Send Form"

Automatically Via the Journals Feature

You can send your patients these assessments automatically over a specific time interval. Click here to learn more about creating a measurement-based care plan via  Journals.

Viewing the Assessment Scores

Once the client receives and completes the form, you can click on the tile on the client timeline to view the scored results.
