How to Get the Public URL of a Consent Form
To get the public URL of an individual consent form, which allows you to integrate the form with your website as well as other uses, follow the steps below. Take note that the steps differ slightly between Standard and Native PDF consent forms.
Standard Consent Forms
- Go to "My Forms" in the top menu bar.
- Click on the name of the consent form you want to add to your website, which opens the form template editor.
- On the left side menu you'll see "Get URL", click this to get the public URL for this specific form.
- Check the option that says "I want this intake form to be accessed through my website".
- If your organization has more than one practitioner, choose which practitioner submitted forms should be routed to.
- Use the given URL to create a link on your website, add one to an email, or for another purpose.
Native PDF Consent Forms
- Go to "My Forms" in the top menu bar.
- Click on the name of the consent form you want to add to your website, which opens the form template editor. This type of consent form is labelled with "PDF" in the box.
- Inside the editor click on "Actions > Get URL" to get the public URL for this specific form.
- Check the option that says "I want this intake form to be accessed through my website".
- If your organization has more than one practitioner, choose which practitioner submitted forms should be routed to.
- Use the given URL to create a link on your website, add one to an email, or for another purpose.
In order for mapped fields to populate forms automatically, you will need to check "Require Email Verification". However, if you're using devices in your office, you can instead choose to "Authenticate the Office Device" to enable this feature.
