Partially Fill Out a Form Before Sending it to Clients

Some of your consent forms may need to be partially filled out before being sent to clients. 

You have two options to address this: Customize the Consent Form or Use Mapped Fields to automatically populate the form.

Customize a Consent Form

  1. Click My Forms.
  2. Open the consent form you wish to edit.
  3. Edit the form as you please.
  4. Make sure your changes are saved. Note: The Saved button on the top left will appear grayed out when it is saved.

Attach the newly customized consent form to a questionnaire

  1. Click My Forms and open the questionnaire to which you want to link the consent form.
  2. Click the Consent Forms button.
  3. Check the box for the customized consent form.
  4. Click Save.
  5. Send the intake package (questionnaire + consent form) to your client:
  6. Click Send to Client. 
  7. Enter the required information.
  8. Click Send Intake Form.

The next time you need to edit the form before sending it out, repeat the steps above.

Use Mapped Fields

  1. Click My Forms.
  2. Open the consent form you wish to edit.
  3. Click Insert Field → Text Box.
  4. Click on the newly created fields, and add a custom mapping to the form. Learn more about mapping fields in Map Consent Form Fields. Learn about how to map custom fields in Map a Custom Field to a Client Profile.
  5. Enter the information in the client's timeline and/or profile.

When the form is sent to the client, the system will automatically fill the fields with information from the client's profile/timeline. If the client is starting the form from a public URL, you must confirm that Require Email Verification is enabled for the auto-fill to work.
