Import Standard Forms

Creating questionnaires and assessments from scratch can be difficult, so PracticeQ offers standardized forms for various business types. 

Browse our standard form library and import forms into your account. The forms can then be used as they are, or modified to suit your specific needs. 

Import Standard Forms

1. Click My Forms.
2. Click the Standard Forms button in the upper right corner.
3. Choose your desired specialty.
4. Click the arrow icon to preview a form in a new window.
5. Click Import for all forms you wish to add to your library.
6. You will see that each form and associated consent forms will appear in your account under My Forms.

Edit a Form

1. Click My Forms.
2. Click the form you wish to edit.
3. Remove, add, and edit the questions to suit your practice's needs.
