How Do I Make My Intake Forms More Attractive?

We know you want to customize your intake form and make it look just right, so our form editor is built to offer you a wide variety of question formats, assuring the form is displayed in a clean, intuitive, and dynamic way. Below shows all of the question types offered for intake questionnaires, be sure to hover over the "example" links to see a sample of what each question type can do.

Once your questionnaire has been created, let’s take a look at some changes that can be made to improve and customize your intake form’s appearance.

Form Layout

Please note that this layout option is for older customers only.

You can choose between 2 different intake form layouts. The classic template with a sidebar on the right, or an alternative more modern view with no sidebar. For the second option, we pushed the buttons and progress indicator to the bottom and made the questions use the full width of the page. This is a better way to display matrix table style questions.

If you would like to switch from the older sidebar layout to the newer wide layout, follow the steps below:

  1. Navigate to "More > Settings".
  2. Select the "Form Layout" tab.
  3. Choose the layout you prefer as shown in the image above.

Hiding "Extras"

If you want less information displaying on your forms, here are some things you can do to further customize them:

Hide Progress Indicator:

Classic progress bar

Full-width progress bar

From inside the form editor:

  1. Navigate to "... > Settings".
  2. Check the "Hide progress indicator" option.

Hide Default Instructions:

From inside the form editor:

  1. Click on "... > Settings".
  2. Check the "Hide default instructions" option.

If you want to write your own instructions or a note to substitute that, just follow these four steps:

  1. Click on the "Add New Question" button.
  2. Move the newly added field for question to the top of the form.
  3. Under "Question Type", pick "Section Title / Note".
  4. Enter new note/instructions under the "Section Text" area.

Custom Header

This can make a huge difference. Your intake form's header can display a logo or other images of your choice, business information, etc.

Here is how you customize the form header for your entire account:

  1. Navigate to "More > Settings".
  2. You are at the right tab, "Form Header". Check the "Enable Custom Header" option.
  3. As you start creating your header, a preview of what it will look like is displayed below. You can use the "header Helper" to assist you.
