Measurement Based Care and Pinned Notes

Please Note: Measurement Based Care questions should not have the Pinned Notes option enabled. Pinning a Measurement Based Care question will prevent the score from updating in the patient chart with subsequent submissions.

Using PracticeQ for Measurement Based Care

In an effort to improve patient outcomes and streamline clinical workflow, PracticeQ makes it east to track assessment scores over time.

By incorporating MBC into your practice, you can gain valuable insights into treatment effectiveness, allowing you to tailor interventions and optimize patient care for your clients.

Follow the guide below to get set-up utilizing scored measures to track patient outcomes.

Enabling the Measurement Based Care Feature.

Go over to this page and enable the Measurement Based Care feature towards the bottom of the page.

Selecting Measures

You may have started out with a library of measures and intakes already included in your account on the "My Forms" page. If not, you can go to the "Standard Forms" button from the "My Forms" page and under the "Mental Health" category, you'll find our list of importable measure.

Don't see the measure that you need? Upload your own and have our team convert it for you. 

We currently support graphing the following measures:

Adding a Scored Matrix to the Progress Graphs

Viewing measure scores on the graph requires specifying which of the scored matrix questions you want to be graphed. This allows you to graph  any questionnaire with a scored matrix.
We also give you defaults to get you going quickly on some of the most used measures including the PHQ-9, GAD-7 and the PCL-5.

1. Go to the questionnaire from which you want to chart the score 

2. From the question that contains the matrix with the scored questions, click the gear icon to the right of the checkbox

3. Enter the Measure Name

Note: If you have the same measure within multiple questionnaires (i.e: PHQ-9 in the Intake and as a standalone), make sure they have the same name (PHQ-9) to have them show on a single graph. If there are more than one measure completed in the same timeframe "biweekly" or "monthly", it will process the most recent one on the graph.

4. You can autofill with common ranges via the "auto fill" button:

5. You can adjust the colors you want the range to apply to under the "Color" column or add a new range if you wish.

6. Click the green "Save" button to confirm the settings.

Now, as clients complete the measure with the scored matrices, the scores will show on the "Progress" tab for the clients. This serves as your MBC dashboard!
From here, you can download the PDF to share out to the client, staff or colleagues.

Accessibility:

Practice is accessible via desktop and mobile browsers. It does not have a stand-alone app. Clients can complete forms via their mobile phone, laptop or desktop computers.

Portal:

The secure client portal that gives your clients a web interface they can access to exchange messages with you and perform various other tasks, such as scheduling appointments, viewing intake forms, paying for invoices, sharing documents, and more.

Customizable Form Reminders:
PracticeQ allows you to set up an automated reminder that is sent your clients when they don't submit a form after a certain period of time has passed. The form reminder can also be programmed to be sent a few days before a client's appointment date.

Administering & Collecting Measures from Clients

PracticeQ allows you to administer measures in many ways:

  1. You can administer the assessment during the visit by including it into your note template
  2. You can send the assessment to the client manually
    1. One way to send manually:
      1. Click the "+" icon on the client timeline and select the desired questionnaire to send
      2. Use the Journal feature to have the system send out the questionnaire automatically at the desired intervals
      3. From the questionnaire, click the "Send to Client" in the top right
  3. You can utilize the Journal feature to automatically send the measures at the desired intervals.
  4. You can also include the scored measures in your intake packet

Reviewing Scores From the Progress Tab

Once the client has completed the questionnaires, the scores will be displayed on the "Progress" tab of the client's profile. This acts as your "MBC Dashboard" for this client.

  1. From the client's profile, go to the "Progress" tab
  2. Choose the desired assessment(s) you'd like to view from the blue dropdown
    1. The scores of the measures will be displayed on the graph below
  3. Choose the displayable dates from the filter on the right
  4. To change how you view time on the graph (x-axis), click the gear icon to edit the "Display By" intervals
    1. You can view the scores by:
      1. Monthly
        1. If there are several in the same timeframe "biweekly or monthly", it will display the most recent one.
      2. Bi-Weekly
      3. By Date

Act

  • Currently, you can enable notifications so you can be notified when your client's complete their assessments. This will notify you to check the submitted form for scores that could lead you to believe the client is experiencing a mental health emergency.
